Please read through the manual first before looking at or using the forms provided. You may find that you don’t have to use a lot of the forms at all, or you might be able to complete a lot of the forms online, without needing the paper forms at all. So, save yourself some time in the long run by using the manual first!
The easiest way to use this kit is to print out the checklist (starting on the next page) and file it in a folder together with copies of the letters / faxes / forms you send out. There is no need to print out the entire kit, keeping it in electronic form means you can use the internet links quickly and easily, and refer to information as you need it.
The forms provided have been collected from organisations on your behalf, and are complimentary bonus to this kit. You have not been charged for the forms, just the information in the manual. The forms are located in the folder titled “Name Change Kit Forms” and can be printed as you need them.
